Job Description

Overview

As a Home Health Occupational Therapist at CenterWell Home Health, you will help patients regain strength, mobility, and independence in their own homes. You'll deliver personalized care focused on rehabilitation, empowering individuals to overcome physical limitations and improve their quality of life.

Key responsibilities

  • Confer with physician and clinical team to develop and revise therapy treatment programs
  • Participate in care coordination and discharge planning activities
  • Train staff and family members on the retraining program for daily living skills
  • Submit evaluation, treatment plans, progress reports, and discharge summaries
  • Provide written instructions and care plans to Occupational Therapy Assistants and Home Health Aides
  • Oversee staff performance and documentation completeness

Required skills

  • Current and unrestricted Occupational Therapy license in North Carolina
  • Valid driver's license and auto insurance
  • Reliable transportation
  • Current CPR certification
  • Home Health experience (preferred)
