Overview

Aya Locums is seeking a Home Health Nurse Practitioner (NP) or Physician Assistant (PA) for in-home health evaluations in West Virginia. The position offers flexible scheduling with no call requirements and the opportunity for both short-term and long-term contracts.

Key responsibilities

  • Conduct pre-scheduled in-home health evaluations for Medicare Advantage, Medicaid, and select commercial plan members
  • Perform comprehensive medication reviews, medical history reviews, and physical evaluations
  • Average 6 to 9 visits per day
  • Complete evaluations using pre-populated, patient-specific forms

Required skills

  • Board Certification required
  • Active Tennessee license required
  • Master's degree required
  • Experience with home health care and patient evaluation
