Job Description

Overview

The Harm Reduction Services Coordinator at Caracole Inc. is responsible for providing community-based interventions and case management services for high-risk individuals who are HIV negative or positive and use drugs. This role includes delivering outreach, education, and direct intervention services, while linking clients to essential resources and advocating for their needs within the community.

Key responsibilities

  • Provide outreach and HIV testing for high-risk individuals who abuse drugs
  • Deliver education on harm reduction and safer drug use practices
  • Facilitate case management intervention services, including service assessments and planning
  • Link clients to HIV/AIDS and substance abuse medical care
  • Serve as an advocate for clients, maintaining communication with providers and the community
  • Maintain accurate client records and meet documentation deadlines
  • Drive testing van to community locations and provide transportation for clients
  • Participate in quality management and team meetings

Required skills

  • Bachelor's degree in public health, social work, or related field, or 4 years of equivalent experience
  • CDCA or CDC certification preferred, HIV Test Counselor certification within 6 months of hire
  • Basic knowledge of HIV/AIDS, substance abuse networks, and community services
  • Ability to work independently and collaboratively with a team
  • Basic computer skills, including Microsoft Office proficiency
  • Strong verbal and written communication skills in English
  • Sensitivity to chronic disease and addiction
  • Ability to travel and work in various community environments
