Overview

Source Medical, LLC. is seeking a Geriatrics Physician Assistant to join their PACE Interdisciplinary Team in Brighton, Massachusetts. This full-time, permanent position involves collaborating with medical doctors to provide comprehensive primary care to participants, focusing on geriatric care and evaluation across various settings.

Key responsibilities

  • Conduct annual and semi-annual physical examinations of participants
  • Evaluate participants in homes, assisted living facilities, and at the center for episodic visits
  • Make nursing home visits as clinically indicated
  • Perform home visits for acute or chronic situations when participants are homebound
  • Participate in After Hours On-Call Coverage
  • Provide health care teaching to patients, families, and caregivers
  • Adhere to APP/MD Collaborative Practice Guide standards
  • Complete accurate and timely documentation
  • Ensure duties are performed in a fiscally responsible manner
  • Maintain high standards of care in all client interactions
  • Travel locally as required
  • Perform other duties as assigned

Required skills

  • Master of Science in Nursing (MSN)
  • Board Certification as a Nurse Practitioner in Massachusetts
  • Active MA license and DEA prescribing authority
  • Current CPR certification
  • Minimum two years of experience with a geriatric population
  • Ability to pass a fit test
  • Knowledge of home visits and nursing home care
