Overview

The Geriatrics Physician Assistant role at GHR Healthcare offers an opportunity to practice autonomously in a hybrid and in-home care setting. Designed for practitioners who want to build meaningful relationships with patients while delivering evidence-based care, this position prioritizes quality over quantity in patient visits.

Key responsibilities

  • Provide independent, evidence-based care for chronic kidney disease and complex patients in their homes
  • Collaborate with physicians while practicing at the top of your license
  • Reduce hospitalizations and improve the quality of life for patients
  • Engage in fewer visits but have deeper conversations with patients
  • Schedule includes 4 days in the field and 1 telehealth day from home

Required skills

  • Experience in geriatrics or chronic care management
  • Knowledge of home health care and telehealth practices
  • Strong patient care and evidence-based practice skills
