Job Description

Overview

The Family Supportive Housing Services Coordinator at Pathways Vermont plays a crucial role in assisting individuals and families exiting homelessness or at risk of becoming homeless. This position involves intensive collaboration with community partners to provide essential resources and support, helping clients gain life skills and access to permanent housing, thereby improving their quality of life.

Key responsibilities

  • Engage in rapport building with participants using a choice-based, participant-centered approach.
  • Manage a caseload of 12 to 15 clients to assess housing needs and provide necessary services.
  • Conduct regular meetings with clients in various settings to ensure ongoing support.
  • Provide training and referrals on financial empowerment, life skills, and addiction recovery.
  • Assist clients in preparing Individual Service Plans (ISP) to set their goals.
  • Advocate and collaborate with agencies on behalf of families.
  • Perform other duties as assigned.

Required skills

  • Knowledge of domestic and sexual violence risks and their relationship to homelessness.
  • Experience working with families and children in home or community settings.
  • Experience with families facing complex needs.
  • Self-reflection and willingness to adopt new relationship practices.
