Job Description

Overview

The Family Supportive Housing Services Coordinator at Pathways Vermont will work closely with individuals and families transitioning out of homelessness or facing homelessness. The role emphasizes a collaborative approach, providing essential housing and service coordination support while focusing on Life Skills training and resource access for clients to enhance their quality of life.

Key responsibilities

  • Build rapport with participants using a choice perspective and participant-centered approaches
  • Manage a caseload of 12 to 15 clients
  • Meet clients regularly to assess housing needs and follow grant protocol guidelines
  • Provide training and referrals regarding financial empowerment and life skills
  • Assist clients in preparing Individual Service Plans (ISP) for goal setting and self-advocacy
  • Engage in advocacy and collaboration with other agencies on behalf of families

Required skills

  • Experience in administration and social services
  • Knowledge of risk factors related to domestic and sexual violence and homelessness
  • Experience working with families and children in home or community settings
  • Familiarity with issues of poverty and substance use
  • Ability to reflect and adapt in client relationships
