Overview

Lutheran Services Florida is seeking a Family Services Case Manager in Fort Myers, Florida, who will provide comprehensive case management services to clients and families. The role focuses on creating and implementing case plans for families while ensuring the safety and well-being of children in their care.

Key responsibilities

  • Accept all assigned cases and conduct family assessments within the required time frame
  • Develop and negotiate case plans with families and relevant stakeholders
  • Visit children in their primary residence regularly and document case activities accurately
  • Provide timely referrals to community resources and maintain contact with service providers
  • Prepare and attend court activities, completing necessary documentation
  • Conduct child safety assessments and prepare safety plans as needed
  • Organize and facilitate family visits and transport children as required
  • Attend appointments, meetings, and training as mandated by the supervisor
  • Function as an on-call Case Manager as scheduled

Required skills

  • Bachelor's degree in a Human Services field, preferably in Social Work
  • At least one year of relevant experience in case management
  • Excellent written and verbal communication skills
  • Familiarity with Microsoft Office programs, particularly Word and Excel
  • Ability to type at least 45 words per minute
  • Valid driver's license and ability to travel extensively
  • Sensitivity to cultural and socioeconomic characteristics of the service population
