Salary Range: $115k $150k (depending on experience)A growing community-based healthcare organization is seeking an NP or PA to join its primary care team. This role supports a diverse patient population and delivers comprehensive outpatient care within a mission-driven, federally qualified health style environment. Multiple openings are available across Brooklyn and Staten Island.Position OverviewThe clinician will provide ongoing primary care services, build long-term patient relationships, and support culturally responsive care for individuals and families. Fluency in Cantonese is strongly preferred to meet the needs of the surrounding communities.This is a full-time, weekday position with a stable schedule. No nights or major weekend commitments!Key ResponsibilitiesDeliver routine primary care for patients of varying ages, including health assessments, preventive visits, chronic disease management, and follow-up care.Provide culturally aligned patient education and support, particularly for Cantonese-speaking individuals.Review diagnostic results, coordinate referrals, and collaborate with the clinical team to ensure continuity of care.Maintain accurate and timely documentation within the electronic health record.Participate in quality improvement and population health initiatives that support community wellness.Work closely with nursing staff, care managers, and other providers to optimize patient outcomes.Support walk-in and same-day care needs as appropriate.QualificationsLicensed Nurse Practitioner (NP) or Physician Assistant (PA) in New York.Experience in primary care, family medicine, or community health strongly preferred; new graduates considered.Cantonese language proficiency highly preferred to serve local patient populations.Strong clinical judgment, communication skills, and a commitment to community-based care.Comfort working in a fast-paced outpatient setting.
