Overview

Position OverviewSeeking a compassionate Nurse Practitioner or Physician Assistant to join a busy, well-established Family Medicine primary care practice. This role provides comprehensive care to patients across the lifespan, from pediatrics through geriatrics, in a collaborative and supportive outpatient setting. Key Responsibilities & Practice Highlights • Perform comprehensive medical evaluations, diagnose conditions, and develop individualized treatment plans for patients of all ages• Manage chronic diseases including diabetes, hypertension, and asthma through ongoing care and medication management• Collaborate closely with physicians, nursing staff, specialists, and other care team members to deliver coordinated, patient-centered care• Accurately document patient encounters, histories, and outcomes using EPIC EMR• Educate patients and families on diagnoses, treatment options, preventive care, and self-management strategies• Stay current with evidence-based guidelines and clinical best practices• Maintain a patient-focused approach emphasizing communication, empathy, and quality outcomes• Average patient volume: 18 patients per day Schedule & Work-Life Balance • 4-day work week• Monday, Tuesday, Thursday, Friday• 8:00 AM 4:30 PM Candidate Requirements • Completion of an accredited Nurse Practitioner or Physician Assistant program• Active and unrestricted Indiana license (NP or PA)• Valid DEA, RN/APRN license (for NP) or PA license• Strong clinical judgment with ability to manage both acute and chronic conditions• Excellent communication and interpersonal skills• EPIC EMR experience preferred
