Overview

Inspire Healthcare is seeking a Board Certified Nurse Practitioner or Physician Assistant for a full-time, permanent role in York, Pennsylvania. The position focuses on providing primary care to an underserved population with a value-based care model, emphasizing personalized care for older adults on Medicare.

Key responsibilities

  • Provide personalized primary care for older adults on Medicare
  • Administer annual wellness visits and health risk assessments
  • Coordinate with the Care Team for population health leadership
  • Ensure documentation is accurate and comprehensive
  • Work closely with operational leadership and report to the Center Medical Director

Required skills

  • Board Certified Nurse Practitioner or Physician Assistant
  • Experience in primary care and patient care
  • Knowledge of ICD-10 coding and hierarchical condition categories (HCC) documentation
  • Skills in health risk assessments and wellness visits
  • Ability to coordinate team efforts for population health management
