Inspire Healthcare is seeking a Board Certified Nurse Practitioner or Physician Assistant for a full-time primary care outpatient position in Brighton, Illinois. The role focuses on providing personalized care to underserved older adults on Medicare, emphasizing health risk assessments and annual wellness visits in a value-based care model.

Key responsibilities

  • Provide primary care to older adults on Medicare
  • Conduct annual wellness visits and health risk assessments
  • Ensure accurate and specific documentation
  • Lead population health initiatives in coordination with the Care Team
  • Manage small patient panels for better focus and consistency
  • Collaborate closely with operational leadership

Required skills

  • Experience as a Nurse Practitioner or Physician Assistant
  • Fluency in Spanish
  • Knowledge of HCC documentation
  • Proficiency in ICD-10 coding
  • Experience in population health management
  • Understanding of wellness visits and health risk assessments
