Overview

Inspire Healthcare is looking for a Board Certified Physician Assistant or Nurse Practitioner for a Full-time permanent Primary Care Outpatient position in Phoenix, Arizona. This role focuses on providing personalized primary care to older adults on Medicare, with an emphasis on maintaining their overall health and well-being, and involves working closely with a care team in a value-based care model.

Key responsibilities

  • Provide personalized primary care for older adults
  • Administer annual wellness visits and health risk assessments
  • Ensure timely documentation with a holistic view of health
  • Lead population health initiatives in coordination with the care team
  • Maintain small patient panels for consistency and focus
  • Report to the Center Medical Director and collaborate with operational leadership

Required skills

  • Board Certification as Physician Assistant or Nurse Practitioner
  • Experience in primary care
  • Understanding of HCC documentation and ICD-10 coding
  • Knowledge of health risk assessments
  • Patient education skills
  • Ability to manage patient care in a value-based model
