Overview

A full-time, permanent position for a board-certified Nurse Practitioner or Physician Assistant is available in Phoenix, NY, to join the PrimeCare team in building a new family medicine practice aimed at delivering comprehensive outpatient care. The clinician will collaborate with physicians to assess patients, develop care plans, and perform essential medical duties as the practice prepares to open in Winter 2026.

Key responsibilities

  • Work as a physician extender in a family medicine practice
  • Collaborate with physicians to assess and diagnose patients
  • Develop plans of care according to practice protocol
  • Obtain complete medical histories from patients
  • Perform physical examinations
  • Order diagnostic tests and make referrals for specialty care
  • See new and established patients in a clinic setting
