Overview

CenterWell and Conviva Senior Primary Care is looking for an experienced Nurse Practitioner or Physician Assistant in Lakeland, Florida, to provide focused primary care for seniors. The role emphasizes a hands-on approach to patient care, addressing both clinical needs and social determinants of health within a multidisciplinary team environment.

Key responsibilities

  • See an average of 15 patients per day
  • Spend time with patients to address their individual needs
  • Thrive in a value-based care model
  • Manage a variety of cases from preventative care to chronic conditions
  • Focus exclusively on direct patient care, including administrative duties related to patient care

Required skills

  • Active, unrestricted NP/PA license in Florida
  • Current board certification as an NP or PA
  • Current DEA license or the ability to obtain one
  • Two or more years of experience as an advanced practice provider seeing patients 65+ or with chronic conditions
  • Familiarity with value-based care model
  • Outpatient senior care experience
  • Knowledge of HEDIS measures and Medicare Advantage
