Overview

Conviva Senior Primary Care is looking for an experienced Float Nurse Practitioner or Physician Assistant to join their team in Pinellas County, Florida. This role focuses on seniors and emphasizes direct patient care within a unique, multidisciplinary care model that aims to improve health outcomes by addressing social determinants of health.

Key responsibilities

  • See an average of 15 patients per day
  • Spend time addressing both clinical and social aspects of patient care
  • Manage a diverse caseload, including preventative care and chronic conditions
  • Perform general administrative tasks related to direct patient care

Required skills

  • Active, unrestricted NP/PA license in Florida
  • Current certification as an NP or PA
  • Board certification
  • Current DEA license or ability to obtain one
  • Two years or more experience with patients aged 65+ or with chronic conditions
  • Familiarity with value-based care model
  • Experience in outpatient senior care
  • Knowledge of HEDIS measures and Medicare Advantage
