Overview

The Family Practice/Primary Care Nurse Practitioner will conduct home-based patient assessments in a float model within Richland County, SC. This role involves performing health assessments, documenting findings, and maintaining communication about patient care without treating or prescribing.

Key responsibilities

  • Conduct 5-6 in-home health assessments per day
  • Perform annual risk assessments and chronic condition reviews
  • Escalate urgent findings to the patient's Primary Care Provider
  • Document all encounters per program guidelines
  • Maintain timely communication regarding escalations
  • Utilize provided tools and resources for documentation and testing

Required skills

  • Active South Carolina nurse practitioner license
  • Board Certified Family Nurse Practitioner
  • Credentialed/enrolled with Medicare
  • Healthcare & Medical knowledge
