Overview

Aya Locums is seeking a Family Practice/Primary Care Nurse Practitioner for home health assessments in Iowa. This role involves conducting in-home evaluations, performing risk assessments, and maintaining patient documentation, with the option for full or part-time schedules.

Key responsibilities

  • Conduct 5-6 in-home health assessments per day
  • Perform annual risk assessments and chronic condition reviews
  • Escalate urgent findings to the patient's PCPA
  • Complete structured in-home evaluations
  • Document all encounters per program guidelines
  • Maintain timely communication regarding escalations
  • Utilize provided tools and resources for documentation and testing

Required skills

  • Active state Nurse Practitioner license
  • Board Certified Family Nurse Practitioner
