Overview

Concord Physicians Solutions is seeking a Geriatrics, Family Medicine, or Internal Medicine Physician for locum coverage in nursing home programs in Cambridge, Massachusetts. The role entails working in a collaborative care environment focusing on post-acute rehabilitation and long-term care management.

Key responsibilities

  • Provide full-time locum coverage in nursing home and rehabilitation facilities
  • Conduct admissions and follow-up visits for short-term rehabilitation and long-term care residents
  • Manage a patient volume of 6-8 patients per day, including new admissions
  • Collaborate with nurse practitioners who handle calls, medication refills, and orders
  • Participate in long-term care follow-ups every four days unless acute issues arise

Required skills

  • Active Massachusetts medical license
  • Massachusetts Controlled Substances Registration
  • Proficient in EMR systems, specifically Epic and PointClickCare
  • Experience in geriatric care and family or internal medicine
  • Ability to work in a collaborative care environment
