Key ResponsibilitiesConduct thorough physical assessments of patients in their home environments to identify individual needs, goals, and barriers to mobility.Develop and implement personalized treatment plans based on assessment findings and patient preferences, ensuring they adhere to best practices and evidence-based approaches.Utilize various therapeutic modalities and techniques to improve patients' physical abilities, such as therapeutic exercises, functional training, and pain management strategies.Collaborate with other healthcare professionals, including Occupational Therapists and Rehabilitation Medicine physicians, to create integrated care plans that address all aspects of a patient's health.Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes, documenting each session accurately and thoroughly in electronic health record (EHR) systems.Provide education to patients and their families about therapeutic exercises, home safety, and injury prevention, fostering independence and health awareness.Participate in regular team meetings and case discussions to address patient care strategies and share insights regarding treatment methodologies and patient responses.Ensure compliance with all legal, ethical, and professional standards related to patient care. Patient Population / Client Focus The clientele at Health Coast Behavioral Center encompasses a diverse range of individuals requiring physical health therapy in home care settings. This includes elderly patients recovering from surgeries or experiencing chronic conditions, as well as those needing rehabilitative assistance after injuries. QualificationsBachelor's degree in Physical Therapy from an accredited institution.Current state licensure to practice as a Physical Therapist in California.Strong foundation in various physical therapy techniques, modalities, and proven methodologies.Exceptional skills in conducting accurate assessments and developing effective treatment plans.Ability to communicate clearly and compassionately with patients and their families.Knowledge of EHR systems and commitment to maintaining high standards of documentation and record-keeping.
