Job Description

Overview

The Director of Social Services at Little Sisters of the Poor will advocate for elderly residents and educate on residents' rights, ensuring they are treated with dignity and respect. The position requires a licensed social worker with a commitment to supporting clients and coordinating services for their admissions to the Home.

Key responsibilities

  • Conduct comprehensive assessments of clients to determine eligibility and needs during the admissions process.
  • Coordinate and facilitate client admissions, ensuring documentation and requirements are completed accurately and timely.
  • Develop individualized care plans in collaboration with clients, families, and interdisciplinary teams.
  • Provide ongoing support and counseling to clients and their families.
  • Maintain accurate and confidential client records in compliance with regulatory standards.
  • Collaborate with healthcare providers, community resources, and other stakeholders to ensure continuity of care.
  • Demonstrate dependability by consistently meeting deadlines and maintaining regular attendance.

Required skills

  • Licensed Social Worker (LSW, LGSW, or equivalent) in the state of Minnesota
  • Experience in the Long Term Care Setting
  • Experience with client admissions processes preferred
  • Strong organizational skills and attention to detail
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a multidisciplinary team
  • Demonstrated dependability and professionalism in previous roles
