Job Description

Overview

The Director of Accountable Care and Clinically Integrated Network (CIN) at Community Health Network will lead the growth, performance, and transformation of providers under value and risk-based reimbursement models. This role involves overseeing network operations, data analytics, and adherence to care policies to enhance healthcare delivery and patient outcomes.

Key responsibilities

  • Lead the creation and operational management of the CIN
  • Oversee network growth and performance metrics
  • Manage data integration and analytics for informed decision-making
  • Ensure adherence to policies and care pathways
  • Support CMS Medicare Shared Savings Program (MSSP) operations
  • Facilitate meetings with leaders and providers to drive strategic discussions

Required skills

  • Bachelor's degree (Master's preferred)
  • 3+ years of health plan experience with a provider perspective
  • Expertise in value-based care principles and health policy
  • Experience in practice redesign, quality improvement, and ACO development
  • 3+ years working knowledge of clinical quality metrics (e.g., RAF, HEDIS)
  • 5+ years in data analytics
  • 5+ years of leadership in practice management and project management
  • 7+ years of leadership in a multi-group practice or integrated healthcare system
  • Preferred experience in a CIN or ACO
