Job Description

Overview

The Director of Accountable Care and Clinically Integrated Network (CIN) at Community Health Network will lead the growth and performance of the CIN under value-based reimbursement models, focusing on creating superior patient care and competitive advantages for providers. This role requires strategic leadership in data integration, analytics, and adherence to healthcare policies to enhance care quality and cost-effectiveness.

Key responsibilities

  • Oversee network growth and performance under value-based care models
  • Lead data integration and analytics initiatives
  • Establish and ensure adherence to policies and pathways for quality care
  • Support CMS Medicaid Shared Savings Program (MSSP) regulatory functions
  • Lead meetings with healthcare leaders and providers
  • Translate data analysis insights into actionable opportunities for improvement

Required skills

  • Bachelor's degree required; Master's degree preferred
  • 3+ years of health plan experience with a provider perspective
  • 3+ years in practice redesign, Patient-Centered Medical Home, and quality improvement
  • 3+ years of working knowledge of clinical quality metrics like HEDIS and NCQA
  • 5+ years of experience in data analytics
  • 5+ years of leadership experience in practice management and provider relations
  • 7+ years of leadership experience in multi-group practices or managed care organizations
