Overview

As a Direct Support Program Coordinator at Sevita, you will provide direct care to clients with intellectual and developmental disabilities while coordinating the development and implementation of their service plans. The role involves training staff, scheduling shifts, and performing administrative tasks, all aimed at empowering individuals to achieve greater independence.

Key responsibilities

  • Assist clients in accessing medical and health-related services
  • Provide direct care to clients receiving services
  • Coordinate the development and implementation of service plans
  • Train and supervise staff
  • Evaluate plan implementation and coordinate services
  • Provide on-call coverage and crisis intervention support
  • Conduct assessments and home visits for referred clients
  • Maintain accurate client and staff reporting records
  • Assist with legal, licensing, and regulatory activities
  • Perform other related duties as required

Required skills

  • High School diploma or equivalent (Bachelor's degree preferred for specific roles)
  • Three years of related work experience
  • Current driver's license and auto insurance
  • Training and certification as required by state
  • Knowledge of client assessment and service plan coordination
  • Experience in staff training and leadership
