Role Description The Direct Support Professional is a full-time, on-site role located in New Haven, CT. This role involves providing hands-on support to individuals with intellectual disabilities, brain injuries, and elderly clients in their daily activities, such as personal care, meal preparation, light housekeeping, and medication reminders in accordance with agency policies and care plans. Responsibilities include implementing individualized support and training plans, assisting with community outings and transportation, and encouraging participation in social and recreational activities. The Direct Support Professional will monitor client well-being, document observations and services provided, and communicate regularly with supervisors, families, and healthcare professionals. The role also includes supporting behavior and safety plans, advocating for client needs, and contributing to a respectful, inclusive, and person-centered environment. QualificationsCandidates should possess skills and experience in Human Services, including supporting individuals with diverse needs in community or residential settings. Candidates should possess strong Interpersonal Skills and Communication abilities to build trust, collaborate with families and teams, and communicate clearly with clients. Candidates should possess knowledge and experience related to Intellectual Disabilities, brain injuries, or aging populations, including person-centered support approaches. Candidates should possess the ability to provide Training and guidance to individuals in daily living skills, community integration, and safety practices. High school diploma or equivalent required; coursework or certification in human services, social work, or related fields is preferred. Previous experience as a Direct Support Professional, caregiver, or similar role in residential or community-based services is beneficial. Ability to perform physical tasks associated with direct care (such as assisting with mobility) and to maintain documentation accurately and promptly. Reliable transportation to and from the New Haven, CT worksite, and availability to work a consistent full-time schedule, including evenings or weekends as needed.
