Overview

The Dialysis Social Worker position at U.S. Renal Care in Hawkinsville, Georgia, involves being a key player on an interdisciplinary team dedicated to supporting patients with kidney disease. Key responsibilities

  • Conduct patient assessments and care planning activities

  • Coordinate communities of support and identify social resources
  • Educate staff on patient psychosocial care and promote teamwork
  • Participate in quality improvement activities and ensure regulatory compliance

Required skills

  • Master's Degree in Social Work
  • Current licensure in applicable state
  • Knowledge of English and ability to communicate effectively
  • Basic computer skills including Microsoft Office
  • Proficiency in clinical applications within 90 days of hire
  • Previous experience in providing social services to dialysis patients preferred
