Job Description

Overview

As a Dialysis Social Worker at U.S. Renal Care, you will be an essential part of a cross-functional team focused on assisting patients with kidney disease in achieving optimal social functioning and psychological adjustment to dialysis treatments. Your role involves advocating for patients, conducting assessments, providing education, and coordinating resources to support their needs.

Key responsibilities

  • Conduct patient assessments and care planning activities
  • Identify and counsel on psychosocial issues
  • Provide patient and family education
  • Coordinate community resources and support for patients
  • Maintain relationships with local, state, and federal agencies
  • Promote teamwork within the interdisciplinary clinic team
  • Participate in clinical and operational improvement processes
  • Ensure compliance with applicable laws and regulations

Required skills

  • Master's Degree in Social Work from a CSWE accredited program
  • Current licensure in applicable state
  • Communication skills in English, both verbal and written
  • Basic computer skills including Microsoft Office
  • Proficiency in USRC clinical applications within 90 days of hire
  • Preferred experience in providing social services to dialysis patients
