Overview

The Community Support Specialist at Communicare provides essential case management and mental health support to clients with serious mental illness and related conditions. The role involves coordinating services, conducting assessments, and advocating for clients to ensure they receive necessary resources and support.

Key responsibilities

  • Obtain and maintain certification to perform Community Support Services
  • Plan and manage productivity requirements effectively
  • Provide case management services and ongoing client assessments
  • Coordinate services with other agencies and provide supportive counseling
  • Assist clients with applications for benefits and housing
  • Support medication management and mental health service access
  • Conduct home visits and provide advocacy and family counseling
  • Maintain a case load and ensure proper documentation
  • Attend meetings and training as required
  • Perform after hours duties as part of the crisis team

Required skills

  • Bachelor's degree in social work, counseling, psychology, or related field
  • Experience with case management and client advocacy
  • Knowledge of mental health and substance abuse counseling
  • Ability to relate to severely mentally ill individuals
  • Valid driver's license and insurable driving record
