Job Description

Overview

The Cognitive Retraining Specialist at Sevita is responsible for providing individual and family therapeutic services to clients with brain and spinal cord injuries as well as other complex challenges. This role involves assessing client needs and developing individualized rehabilitation plans in a collaborative team environment.

Key responsibilities

  • Maintain a caseload of clients and provide direct client services
  • Evaluate client needs and develop individual rehabilitation plans (IRP)
  • Create, coordinate, and evaluate service plans through partnerships with stakeholders
  • Provide case management with attention to insurance needs
  • Assess referred clients and lead development of service plans
  • Participate in team conferences and lead conferences as needed
  • Complete documentation accurately and timely
  • Maintain and update client clinical records and progress notes
  • Engage in the on-call process as scheduled
  • Perform other related duties as required

Required skills

  • Bachelor's degree plus CSW or teaching certification (Master's preferred)
  • Minimum one year experience with clients having developmental disabilities or mental health issues
  • Licensure and/or certification in field as per site contract and state standards
  • Current driver's license, car registration, and auto insurance
