We are looking for a dedicated professional with experience coordinating services for individuals with developmental disabilities and a strong understanding of the SLS model. Vision Life provides Supported Living Services (SLS) to adults with developmental, intellectual, and behavioral disabilities, empowering them to live independently while promoting dignity, choice, and meaningful community inclusion. The ideal candidate will have experience working within an SLS program, collaborating with Regional Centers, supporting IPP/ISP implementation, coordinating staffing and client services, maintaining compliance with Title 17 requirements, and ensuring accurate, timely documentation. As a Client Case Coordinator, you will serve as the primary point of contact for assigned clients, families, Direct Support Professionals (DSPs), Regional Center Service Coordinators, and the interdisciplinary team. You will be responsible for coordinating client services, monitoring service delivery, assisting with staffing and scheduling, conducting home visits, completing required documentation, addressing client concerns, and helping ensure each individual receives high-quality, person-centered support. This position requires exceptional organizational skills, the ability to manage multiple priorities, strong communication, and a genuine passion for helping individuals achieve greater independence and an enhanced quality of life.Responsibilities Under the direction of the SLS Program Supervisor and Program Director, duties include but are not limited to:Coordinate and schedule client services and staff coverageEnsure authorized service hours are implemented appropriatelyConduct intake assessments and participate in client screenings and referralsCreate IPP, ISP and SIR reports independentlyProvide case management services using individualized support approachesMentor, support, and help manage Direct Support Professionals (DSPs)Complete monthly, quarterly, and annual reports and documentationParticipate in home visits and community-based support when necessaryProvide crisis intervention and behavioral support when neededMaintain client files, at-home binders, and updated documentationCollaborate with multidisciplinary teams and community resourcesCoordinate with Regional Centers, vendors, behavioral teams, and outside professionalsParticipate in on-call support rotation in accordance with company policyDeliver exceptional customer service and client-centered careQualificationsHigh5 school diploma/GED with relevant experience requiredAssociate's or Bachelor's degree in Human Services, Psychology, Social Work, or related field preferredExperience working with individuals with developmental disabilities, intellectual disabilities, and behavioral disabilities preferredKnowledge of Regional Center and DDS protocols is a plusStrong communication, organizational, and critical thinking skillsAbility to manage multiple tasks independently and professionallyValid California Driver's License requiredReliable transportation and current auto insurance requiredDOJ/Fingerprint clearance requiredCPR/First Aid certification preferredCPI certification is a plusBilingual or ASL skills are a plusSchedulePart TimeMonday-WednesdayFlexible availability may be required based on client needsPay & BenefitsCompetitive pay: $25-$30 per hour DOEMileage reimbursementOpportunities for growth and advancementSupportive team environmentMeaningful and rewarding work experienceTravel Requirements This position requires travel throughout the Pasadena and surrounding Los Angeles County areas to support clients in community and residential settings.
