Overview

The Chaplain at Catholic Health Services serves as a spiritual counselor and community liaison for patients in hospice care. This role involves creating spiritual care plans, providing counseling, and coordinating spiritual services in conjunction with an interdisciplinary team, while also ensuring proper documentation and support for patients and families.

Key responsibilities

  • Assess spiritual factors related to patient care and develop a spiritual care plan for patients and families.
  • Provide documentation of care and services on a daily basis.
  • Administer sacraments or arrange for their administration as requested.
  • Provide individual and group spiritual support to patients and families.
  • Act as a liaison with the local religious community for patient visits.
  • Participate as an active member of the interdisciplinary team addressing spiritual matters.
  • Educate the religious community about collaboration with hospice care.
  • Maintain effective communication with team members and provide orientation to new employees.
  • Manage a weekly schedule of patient appointments and follow-ups.
  • Perform other duties as assigned and necessary for interdisciplinary care.

Required skills

  • Master's Degree or Doctorate in Divinity, Theology or related field
  • Membership in A.C.P.E. or N.A.C.C. desired
  • Four units of Clinical Pastoral Education (CPE) preferred
  • 1-2 years of experience in hospice care or related area
  • Knowledge of hospice levels of care and Medicare/Medicaid benefits
  • Office/clinical software proficiency
  • English language proficiency
