Overview

The Chaplain for Catholic Hospice acts as a community liaison for patients, providing spiritual counseling while collaborating with the Interdisciplinary Team. Responsibilities include assessing spiritual needs, creating care plans, administering sacraments, and coordinating spiritual services in accordance with patient beliefs.

Key responsibilities

  • Assess spiritual factors related to patient care and intervene appropriately
  • Establish spiritual care plans for patients and families
  • Provide spiritual counseling that aligns with patient and family beliefs
  • Administer sacraments and perform funeral services as requested
  • Coordinate spiritual services with the interdisciplinary team
  • Educate the religious community on hospice and clergy collaboration
  • Maintain effective communication with interdisciplinary teams
  • Document patient preferences and services provided
  • Participate in on-call rotations and manage patient appointments
  • Comply with all policies and regulations

Required skills

  • Master's or Doctorate Degree in Divinity, Theology or related field
  • Membership in A.C.P.E. or N.A.C.C.
  • Four units of Clinical Pastoral Education (CPE) preferred
  • 1-2 years of related experience, preferably in hospice care
  • Knowledge of computer office/clinical software
  • Ability to read, write and understand English
  • Comfort dealing with terminal illness
