Overview

The McNabb Center is seeking dedicated Case Managers and Counselors at the bachelor's level to provide essential mental health and social services in Claiborne, Cocke, Hamblen, and Sevier Counties, Tennessee. This role involves coordinating care, providing case management services, and offering therapeutic support to both adults and children while working collaboratively with community agencies.

Key responsibilities

  • Provide case management services using a social model approach
  • Facilitate weekly case management sessions and therapeutic groups
  • Offer information, referral, advocacy, and coordination with community agencies
  • Participate in weekly treatment team meetings
  • Monitor medication and provide crisis intervention as needed
  • Maintain complete and timely documentation per agency standards
  • Embrace recovery-oriented values including empowerment and continuity of care
  • Coordinate care across various providers and develop individualized intervention plans
  • Serve as a liaison between schools, homes, and healthcare systems
