Overview

Case Manager - Superior Futures for Youth (Michigan) Lutheran Social Services of Wisconsin & Upper Michigan (LSS) Lutheran Social Services of Wisconsin and Upper Michigan (LSS) is seeking a full-time Case Manager to join our growing Superior Futures for Youth program. This program serves youth experiencing homelessness across Delta, Menominee, and Schoolcraft Counties, with this position primarily supporting Dickinson and Iron Counties. This role provides housing stabilization and supportive case management services to youth ages 12-24, helping them achieve long-term stability and independence. The ideal candidate is passionate about youth services, highly organized, and comfortable working independently in community settings. Travel up to 75% throughout the service area is required. The ideal candidate would reside within one of the counties served. This is a full-time (40 hours/week), benefits-eligible position. Position Summary Under the direction of program leadership, the Case Manager delivers professional-level social work and case management services to individuals and families. This role involves assessing client needs, coordinating services, and providing ongoing support to help youth achieve stable housing and improved well-being. Key ResponsibilitiesConduct comprehensive assessments through interviews, collateral contacts, and other evaluation methodsDevelop individualized case plans in partnership with youth and ensure delivery of agreed-upon servicesProvide ongoing case management and coordinate housing, social, and mental health servicesBuild and maintain collaborative relationships with community partners and service providersMaintain accurate and timely case records, documentation, and required reportsParticipate in court proceedings or hearings as requiredPromote program services and accept referrals from community sourcesParticipate in staff development, trainings, team meetings, and professional learning opportunitiesSupport quality improvement efforts and contribute to a positive team environmentRepresent LSS in the community, including outreach and engagement activitiesPerform additional duties as assigned Qualifications Required: Bachelor's degree in Social Work, Sociology, Psychology, or a closely related field (or equivalent combination of education and experience)Minimum of two (2) years of professional experience in human services or related fieldKnowledge of case management practices, social service systems, and community resourcesUnderstanding of social, behavioral, and mental health challenges affecting youthStrong organizational, communication, and documentation skillsAbility to work independently and manage time effectivelyValid driver's license, reliable transportation, and ability to meet LSS driver requirements Preferred: State certification such as Certified Social Worker (CSW), Certified Professional Counselor (CPC), or equivalent Travel RequirementsTravel within service counties up to 75% of the timeOccasional overnight travel may be required Benefits & Perks LSS offers a comprehensive benefits package to support employees' well-being and professional growth:Public Service Loan Forgiveness (PSLF) eligibilityMedical, dental, and vision insuranceFlexible spending accounts (health and dependent care)Mileage reimbursementGenerous paid time off (PTO)10 paid holidays annually403(b) retirement plan with employee contributionsCommitment to annual compensation review and increasesEmployee Assistance Program (EAP)Employee recognition and service awards Equal Opportunity Employer Lutheran Social Services of WI & Upper MI is an Equal Opportunity/Affirmative Action employer.
