Overview

The Care Manager is a crucial leadership position in delivering high-quality, client-centered care for seniors in Columbus, Ohio. This role involves coordinating, supervising, and implementing care services while supporting caregivers and building strong relationships with clients and families.

Key responsibilities

  • Develop and update individualized Care Plans based on client needs
  • Conduct new client assessments and introductions
  • Visit clients in various care settings
  • Supervise and coach caregivers to ensure quality care delivery
  • Provide ongoing training to caregivers
  • Conduct quality assurance visits
  • Address performance concerns and administer discipline
