Overview

The Care Manager (BH Licensed) position at Molina Healthcare involves supporting care management and coordinating care for members with behavioral health concerns. The role requires collaboration with a multidisciplinary team to ensure members receive integrated care and support in achieving their health goals.

Key responsibilities

  • Complete comprehensive behavioral health assessments and determine eligibility for care coordination.
  • Develop and implement care plans in collaboration with members and healthcare professionals.
  • Conduct telephonic, face-to-face or home visits as needed.
  • Monitor care plans for effectiveness and document interventions.
  • Maintain an ongoing caseload for regular outreach and management.
  • Promote integration of services including behavioral health and community resources.
  • Facilitate interdisciplinary care team meetings.
  • Use motivational interviewing techniques to support and educate members.
  • Assess for barriers to care and provide necessary coordination.
  • Consult with peers as needed.

Required skills

  • At least 2 years of healthcare experience, preferably in behavioral health.
  • Licensed behavioral health clinician (LCSW, LMH, LMFT, LMSW or equivalent).
  • Valid and unrestricted driver's license and reliable transportation.
  • Experience working with severe mental health concerns and substance use disorders.
  • Knowledge of whole person care principles and discharge planning coordination.
  • Data entry skills and experience with clinical platforms.
  • Excellent verbal and written communication skills.
  • Proficiency in Microsoft Office Suite.
