Overview

Job Description: Medi Home Health & Hospice, a proud member of the Medical Services of America family, currently seeks an experienced Bereavement Coordinator for our Hospice care patients and families in the Elizabeth City, NC community. As a member of the multidisciplinary team, the Bereavement Coordinator will: Ensures patients and families receive an initial bereavement assessment within 5 days of the start of care visit. Develops and updates the bereavement plan of care to include the type of bereavement services to be offered, frequency and documented needs and goals based on the initial and ongoing assessment of the family's needs and desire for ongoing support. Provides bereavement counseling as related to the emotional, psychosocial and spiritual support and services provided before and after the death of patients to assist with issues related to grief, loss and adjustment. Documentation in the medical record reflects the bereavement assessment is updated as appropriate during the patient's hospice stay, at the time of death and during the provision of bereavement follow up and care. Educates patients, families, staff members, facility staff and the community about grief and loss and the impact it can have on various aspects of their lives. Provides group and/or educational activities for community organizations. Contributes to team effort by accomplishing related results as needed.Why Choose MSA? Experience or education in grief and loss counseling required. Minimum of one year experience in hospice or other health related field preferred.
