Overview

The Assistant Clinical Manager (ACM) is a Registered Nurse leadership position focused on delivering safe and high-quality home health and hospice care. This role involves providing clinical oversight, supervising staff, and coordinating care delivery within community-based services, while ensuring compliance with organizational policies and regulations.

Key responsibilities

  • Support Clinical Managers and Directors in delivering home health and hospice care
  • Provide clinical oversight during assigned coverage, including weekends and holidays
  • Supervise and schedule designated staff
  • Coordinate care delivery and handoffs
  • Serve as a point of escalation for clinical and operational concerns
  • Collaborate with interdisciplinary and administrative teams to ensure continuity of care

Required skills

  • Associate or Bachelor's degree in Nursing
  • Current, unrestricted RN license in state of practice
  • BLS certification
  • Hospice and Palliative Nursing certification (preferred)
  • Three years' experience as a Registered Nurse in Home Health and/or Hospice/Palliative care
  • Two years progressive leadership/supervisory experience
  • Knowledge of hospice/home health regulations and standards of practice
  • Effective written and verbal communication skills
