Overview

The Assertive Community Treatment Team Services Coordinator at Pathways Vermont is a full-time position based in Chittenden County, aimed at providing compassionate and community-based mental health services. The role requires collaboration with clients who experience mental health challenges and histories of homelessness, focusing on their stability and reintegration into the community.

Key responsibilities

  • Build rapport with clients using a client-centered, harm reduction approach
  • Collaborate on comprehensive and Medicaid-compliant service plans with clients
  • Assist clients with housing navigation and moving in processes
  • Maintain an understanding of community resources and facilitate access for clients
  • Provide one-on-one support on life skills, health maintenance, and budgeting
  • Advocate for clients in accessing benefits and community services
  • Participate in 24/7 on-call services rotation with team members
  • Complete program documentation and maintain written records

Required skills

  • Experience in mental health or harm reduction services
  • Knowledge of issues facing individuals with mental health and substance use challenges
  • Excellent communication, writing, and computer skills
  • Strong interpersonal skills and the ability to work with marginalized populations
  • Independent problem-solving abilities
  • Knowledge of community resources for mental health and substance use
  • Valid driver's license and reliable vehicle
