Job Description

Overview

The Assertive Community Treatment (ACT) Team Services Coordinator at Pathways Vermont is responsible for providing flexible, compassionate, community-based services aimed at promoting stability and recovery for individuals facing mental health challenges and homelessness. The role involves working as part of an interdisciplinary team, collaborating with clients to develop and maintain service plans, and advocating for client needs within the community.

Key responsibilities

  • Engage in relationship building and rapport with clients using a client-centered, harm reduction approach
  • Collaborate with clients to create and update comprehensive service plans
  • Assist clients in navigating housing options and life skill development
  • Develop knowledge of community resources and assist clients with referrals
  • Maintain detailed records and documentation of client interactions
  • Provide one-on-one support and counseling on various life skills
  • Coordinate and monitor referrals to community services
  • Advocate for clients regarding benefits and entitlements
  • Participate in on-call services rotation
  • Perform other duties as assigned

Required skills

  • Experience in mental health or harm reduction services
  • Understanding of issues faced by individuals with mental health challenges
  • Excellent communication and interpersonal skills
  • Strong writing and computer skills
  • Ability to develop work priorities independently
  • Problem-solving skills
  • Knowledge of community resources for mental health and substance use
  • Valid driver's license and reliable vehicle
  • Ability to lift up to 50 pounds
  • Experience or education in nursing, substance abuse support, or benefits coordination is ideal
