Job Description

Overview

The Assertive Community Treatment Team Services Coordinator at Pathways Vermont is responsible for providing community-based mental health services to individuals facing mental health challenges. This role involves working as part of an interdisciplinary team to promote stability, recovery, and reintegration for clients, with a focus on supportive and compassionate care.

Key responsibilities

  • Engage in relationship building and rapport with clients
  • Provide one-on-one support and counseling on life skills and housing concerns
  • Assist clients with navigating housing paths and landlord disputes
  • Educate and advocate for clients regarding benefits and entitlements
  • Maintain written and computerized records and compile reports
  • Attend annual All Staff and Programmatic Retreats
  • Perform other duties as assigned
