Overview

The Advanced Practice Practitioner (NP/PA) at Grace at Home is responsible for delivering comprehensive patient care through in-person and telehealth visits, ensuring proper clinical management, and collaborating with multidisciplinary teams. This role aims to enhance patient outcomes and fulfill the organization's commitment to high-quality care in community settings, focusing on urban and rural populations.

Key responsibilities

  • Conduct patient visits for comprehensive care, including follow-up visits and transitions of care.
  • Practice value-based care focused on quality improvements and patient satisfaction.
  • Manage acute and chronic conditions and develop individualized care plans with multidisciplinary teams.
  • Participate in interdisciplinary meetings to enhance care coordination.
  • Prescribe medications and therapies within scope of practice.
  • Maintain accurate documentation in the EMR and ensure compliance with regulations.
  • Support chronic disease management and health education for patients and families.
  • Provide on-call backup support to the nurse line as needed.
  • Engage in quality improvement initiatives and professional development.

Required skills

  • Master's degree in nursing or related field
  • Active NP/PA license required
  • 2+ years of direct patient care experience preferred
  • Experience with Medicare/Medicaid and chronic conditions
  • Excellent verbal and written communication skills
  • Strong analytical and critical thinking abilities
  • Proficiency in Microsoft Office and EMR systems
  • Ability to work in a team-oriented and fast-paced environment
  • Good judgment and ethical standards
