Overview

The Advanced Practice Practitioner (NP/PA) at Grace at Home is responsible for delivering high-quality patient care through in-home and telehealth visits, focusing on innovative health solutions for high-needs communities. Reporting to the Clinical Manager, the role emphasizes effective collaboration and comprehensive management of both acute and chronic conditions while striving to improve member outcomes.

Key responsibilities

  • Conduct patient visits in-home and via telehealth, including annual wellness visits and follow-up care.
  • Practice value-based, population health care focusing on quality and patient satisfaction.
  • Collaborate with multidisciplinary teams to create and implement individualized care plans.
  • Prescribe medications and lifestyle interventions within scope of practice.
  • Maintain accurate documentation in the EMR and ensure compliance with regulations.
  • Participate in quality improvement initiatives and professional development.
  • Provide on-call backup support and participate in daily huddles with the care team.

Required skills

  • Master's degree in nursing or related field
  • Active NP/PA license in the state of hire
  • 2+ years of direct patient care experience, preferably in community-based care
  • Experience with Medicare/Medicaid and treating complex chronic conditions
  • Excellent verbal and written communication skills
  • Strong analytical and critical thinking abilities
  • Proficiency in Microsoft Office applications
