Overview

The Advanced Practice Practitioner (NP/PA) at Grace at Home is dedicated to improving the health and well-being of high-needs communities through personalized care, both in-home and via telehealth. This role involves ensuring effective clinical management and delivering high-quality care to patients while supporting their unique needs and backgrounds.

Key responsibilities

  • Conduct patient visits in-person and via telehealth, including assessments and follow-up care.
  • Practice value-based, home-centered care focused on quality and patient satisfaction.
  • Manage acute and chronic conditions collaboratively with multidisciplinary teams.
  • Participate in team meetings to enhance patient care coordination.
  • Prescribe medications and lifestyle interventions within the scope of practice.
  • Maintain accurate documentation in the EMR and comply with clinical regulations.
  • Engage in quality improvement initiatives and professional development.

Required skills

  • Master's degree in nursing or related science field
  • Active and unrestricted NP/PA license
  • 2+ years of direct patient care experience as a Nurse Practitioner
  • Experience with Medicare/Medicaid
  • Excellent verbal, written, and presentation skills
  • Strong analytical and communication skills
  • Proficiency in Microsoft applications and willingness to learn new technologies
