Job Description

Overview

The Advanced Practice Practitioner at Grace at Home will conduct patient visits and collaborate effectively to ensure high-quality patient care delivery both in-person and via telehealth. This role focuses on comprehensive care management aimed at improving patient outcomes, particularly for high-needs individuals in urban and rural communities.

Key responsibilities

  • Conduct in-home and virtual patient visits, including wellness checks and assessments.
  • Manage acute and chronic conditions in collaboration with multidisciplinary teams.
  • Participate in team meetings for enhanced patient care coordination.
  • Prescribe treatments and lifestyle interventions within the scope of practice.
  • Provide on-call support for the nurse line in licensed states.
  • Maintain accurate documentation in the EMR.
  • Ensure compliance with clinical ethics and regulations.
  • Participate in quality improvement initiatives and professional development.

Required skills

  • Master's degree in nursing or related field
  • Active NP/PA license in the state of hire
  • 2+ years of experience in direct patient care, preferably community-based
  • Experience with Medicare/Medicaid and managing complex chronic conditions
  • Proficiency in Microsoft Office applications
  • Strong analytical and communication skills
