Job Description

Overview

Glens Falls Center is seeking a Registered Nurse for the role of Admissions/Wound Nurse in Queensbury, NY. This position involves overseeing the admission process for new residents and those returning from hospitals, as well as assisting in the development and management of the nursing department under the direction of the Director of Nursing.

Key responsibilities

  • Initiate admission assessments and initial care plans for new residents and hospital returns.
  • Coordinate the admission process with the interdisciplinary team.
  • Audit records for compliance with facility standards.
  • Participate in discharge planning when necessary.
  • Review and evaluate residents' care plans as conditions change.
  • Perform charting according to policy and procedure.
  • Complete MDS, care plans, and other assigned documents.
  • Supervise or direct resident care as needed.

Required skills

  • Active NYS Registered Nurse License
  • Prior experience as an admissions nurse in LTC settings
  • Excellent clinical assessment skills
  • Strong leadership and supervisory skills
  • Proficiency in MS Office (Excel, Word, Outlook)
  • Long-Term Care experience preferred
  • Excellent communication and interpersonal skills
