Overview

The Admissions Coordinator at Catholic Hospice is responsible for managing telephone inquiries related to hospice services and coordinating the scheduling of admissions evaluations. This role requires effective communication with various stakeholders, including referral sources and interdisciplinary teams.

Key responsibilities

  • Handle telephone inquiries about hospice services
  • Coordinate scheduling of referrals for admission evaluations
  • Obtain written consent from patients or family members when criteria for admission are met
  • Set appointments for Admission Nurses
  • Verify demographic information and next of kin
  • Communicate with referral sources regarding appointments and admission details
  • Contact patients/families within 24 hours to explain services
  • Document patient/family services accurately
  • Adjust work assignments based on changing needs
  • Ensure compliance with hospice policies and regulations

Required skills

  • High School Diploma or equivalent
  • 1-2 years of clerical or administrative experience
  • 1-2 years of experience with healthcare admissions process (preferred)
  • Hospice experience (strongly preferred)
  • Knowledge of computer office/clinical software
  • Effective communication skills
  • Ability to handle sensitive information and maintain confidentiality
