Job DescriptionJob Description NEW WEEKLY PAYROLL DEPARTMENT: ActivitiesSummaryResponsible for ensuring the development, organization and coordination of living center and community resources to provide comprehensive Activity Services and programs that meet the needs and interests of each resident.Essential job duties and responsibilitiesInterview and assess all residents prior to the initial Care Plan Conference; document this information in the medical record, develop an individual recreation plan based on the assessment and participate in Interdisciplinary Care Plan meetingsUpdate assessments and plans as needed and required by state or federal regulationsDevelop monthly recreation program calendars that reflect and meet the needs of facility residentsCommunicate facility programs to residents, staff, family and volunteersManage facility Volunteer ProgramMaintain departmental documentation that reflects services provided and resident progress towards goalsIn coordination with social services facilitate the residents in the organization and continued development of a Resident's CouncilMake job assignments and set prioritiesServe as member of QAA committeeCODE OF CONDUCTMust adhere to the Company's Code of Conduct and Business Ethics policy including documentation and reporting responsibilities.QualificationsAs required by state, currently licensed or registered in state of practice as Activities Director, Social Worker, Occupational TherapistHigh school diploma or equivalentCNA certificationMinimum two (2) years' experience in social or recreational program within the last five (5) yearsMinimum one (1) year full-time experience in health care activities programCertification as Therapeutic Recreation Specialist, preferredMust be capable of maintaining regular attendanceKNOWLEDGE, SKILLS, ABILITIES & BEHAVIORSMust be able to maintain confidentiality regarding patient and company proprietary informationMust have the ability to relate professionally and positively and work cooperatively with patients, families, and other employees at all levelsPhysical and Sensory.
